JOB SUMMARY
To be part of Recovery Options Made Easy, Community Re-Integration Team. This team serves patients being discharged from the Buffalo Psychiatric Center to the community. Will provide medication education and teach clients skills regarding self-care of chronic diseases. Works under the supervision of the CEO / Program Manager for Erie/Niagara Counties, for Recovery Options Made Easy.
Under the general supervision of the Program Manager, the Recovery Guide engages with referred individuals to coordinate activities to form trust-based relationships that foster hope for positive change and personal empowerment leading to successful integration back into the community. The Guides work closely with participants to address problems and answer questions, gather information, advocate and connect participants to resources and to the community. Guides assist participants in goal setting and work toward developing skills. The Guides will share and discuss common peer experiences when appropriate. Guides will assist in building a sense of community and assist participants to create meaningful lives in the community. Guides will be responsible for planning, organizing, developing, leading and facilitating group activities.
JOB SUMMARY
The RN will work with Peer Staff, all medical staff and mental health professionals to provide individuals with the essential care needed upon arrival at the ICR. The RN will provide a comprehensive screening for physical health conditions, risk assessment, medication management, and on-going communication and collaboration with all staff and the individual's mental health providers.
The position of Housing Specialist I assesses, refers and links individuals with mental illness to services including but not limited to housing, treatment, and community services. With a working knowledge of the resources in the community to meet the consumer’s needs, the Housing Specialist I assist in providing housing support services to persons receiving government rental stipends in all Recovery Options programs. This may include but is not limited to assisting in finding and securing housing in the consumer’s community of choice and assisting the consumer in maintaining the housing
The Program Coordinator will report to the Clinical Director and provide direct oversight of all Recovery Options Made Easy’s Clinical services by planning, directing, and coordinating the daily functions and future development of peer-operated programs that support individuals experiencing mental health crises and/or emotional distress to stay out of hospital emergency rooms (for behavioral health reasons) and/or psychiatric inpatient programs. Also provides planning and supervision for CORE HBICI & CFTSS services.

The Peer Companion will be available to all guests staying at the Respite Housing Center, to provide peer support and companionship and be available to instill hope, support wellness and demonstrate that recovery is real and does happen. The Peer Companion should discuss the WRAP Plan, Advanced Directives and help guests set goals and engage in community and social events.
To reduce instances of re-hospitalization among peers being discharged from inpatient settings by providing transitional mental health peer support and increased engagement with community based services and natural supports. Not a remote position.

The INSET Family Liaison functions as an advocate and offers support of INSET participant and/or their family (of choice) if indicated by the participant. The Family Liaison will offer services such as advocacy, connecting families with services and referral, and one-on-one support that reflects the rights of self-determination, choice, shared decision-making, and collaboration from a trauma-informed and culturally curious framework. A flexible schedule and on call hours are required. The INSET Family Liaison reports to the INSET Director. Must have Family Specialty (CRPA-F).

These services provide the training and support necessary to ensure engagement and active participation of the youth in the treatment planning process and with the on-going implementation and reinforcement of skills. Services are delivered in a trauma informed, culturally and linguistically competent manner. The YPA will provide structured, scheduled activities that emphasize the opportunity for the youth to expand the skills and strategies necessary to move forward in meeting their personal, individualized goals, develop self-advocacy skills and to support their transition into adulthood.
Per Diem position: Hourly Rate of $19.00 + Mileage

PSR Services in our CFTSS Program are designed to restore, rehabilitate and support an child's/youth's developmentally appropriate functioning as necessary for the integration of the child/youth as an active and productive member of their family and community with the goal of achieving minimal on-going professional intervention. Services assist with implementing interventions on a treatment plan to compensate for, or eliminate, functional deficits and interpersonal and/or behavioral health barriers associated with a child/youth's behavioral health needs. Activities are hands on and task oriented, intended to achieve the identified goals or objectives as set forth in the child/youth's individualized treatment plan.
Per Diem position, $19.00 per hour + mileage
Family Peer Support Services (FPSS) for our CFTSS Program are an array of formal and informal activities and supports provided to families caring for/raising a child who is experiencing social, emotional, mental, developmental, substance use, and/or behavioral challenges in their home, school, placement, and/or community. FPSS provide a structured, strength-based relationship between a Family Peer Advocate (FPA) and the parent/family member/caregiver for the benefit of the child/youth. Services are delivered in a trauma informed, culturally and linguistically competent manner. The FPA must be a New York State Credentialed Family Peer Advocate (FPA) or a Certified Recovery Peer Advocate with a Family Specialty (CRPA-F).
Develops and implements treatment plans for patients with varying disabilities; works as part of a multidisciplinary team providing quality healthcare to all patients in accordance with the principle of ethics and code of professional conduct. Works under the supervision of the Program Coordinator of Erie/Niagara Counties and the Director of Housing and Peer Services for Recovery Options Made Easy.
